The Accommodation Hub is a countywide team working alongside the locality and countywide learning disability & autism and physical and sensory impairment teams. Our role is to support the teams to source accommodation options for adults who have been identified as needing to move.

The move on workers works alongside the allocated workers to identify and source suitable accommodation.

Having access to the right accommodation setting is a key foundation to living a happy and healthy life. There can be many barriers to moving on from temporary accommodation or accommodation that is no longer appropriate, particularly to a more independent setting. Having dynamic and passionate individuals who can support people with disabilities to move on to the setting that is right for them. Supporting our front-line practitioners with some of the practical tasks people need support with to move.

To provide a knowledgeable, practical and personalised service for individuals with disabilities to move on from their current accommodation, working with service users to find and secure an appropriate home. Linking in with colleagues from the social work teams, care providers, health and housing where appropriate.

Responsibilities:

  • Building strong, effective relationships with clients, families and carers providing practical advice, support and, where necessary, acting as an advocate. Enabling vulnerable adults to live healthily and as independently as possible in the right accommodation for their needs.
  • Contribute to the development of care and support plans that gain commitment to solutions to meet individual needs.
  • Work collaboratively across the service, with internal and external professional partner agencies to deliver joint objectives and ensure improved outcomes for vulnerable adults.
  • Provide information and advice relevant to needs of the client (with a focus on housing, benefits agencies, employment, community activities).
  • Responsible for highlighting issues and working with colleagues to implement plans to safeguard vulnerable adults.
  • Specific individual and shared targets and objectives are defined annually within the performance management framework.

Skills, Knowledge and Experience

  • Experience of providing advice and support on housing matters to vulnerable individuals.
  • Experience of working with Learning Disabilities and/or Autism as well as people with Physical and Sensory Impairments would be an added advantage.
  • Excellent communication skills with the ability to problem solve, negotiate and seek solutions to challenging and complex situations.
  • A knowledge of person-centred planning to support the development of networks for people to maximise independence and wellbeing.
  • Ability to work collaboratively, build positive relationships and networks across both internal and external teams and multiple stakeholders focusing on the best outcomes for the individual.

The role requires you to be mobile throughout a wider operational area including travelling around the county of Essex and beyond, transporting service users and their belongings and to be able to do so in an emergency. Driving licence and car or the ability to meet the mobility requirements of the role through other means is essential.
