Move On Facilitator

£27,655 to £32,536 per annum

This is an exciting, fixed term contract or secondment opportunity until 31st March 2026 supporting people through a recovery pathway towards a move into independent housing.
Within a review of Mental Health Supported Accommodation, all partners; Commissioners (Health and Social Care), Operations (Essex County Council and Essex Partnership University NHS Foundation Trust) and Commercial colleagues acknowledge that moving individuals on from Supported Accommodation-based services is a key issue.
Issues and barriers can lead to significant delays in the system, cost pressures, long periods of time when there appears to be no activity taking place and the potential for people’s recovery to deteriorate.
The postholder will work in partnership with various organisations to support resolutions, address barriers, help speed up processes and promote viable move on options for people to successfully live independently and also for people who are ready to be discharged from hospital inpatient settings.
About the Role
To provide a knowledgeable, practical and personalised service for people moving on from Mental Health Supported Accommodation. Working with people and supported accommodation providers to source and secure appropriate homes ensuring that housing issues and daily independent living needs are explored and included in a person’s support plan from the outset.
Accountabilities
- Work with people (manage a caseload) co-ordinating stakeholders required for people to successfully move on from MH Supported Accommodation
- Develop relationships and liaise with supported housing providers, private landlords and housing teams within the City, Districts and Boroughs
- Identifying and sourcing a range of accommodation options across the housing market
- Promoting viable move-on options for people currently residing in supported housing with clear communication regarding move on and discharge from statutory services
- Supporting people through a move-on process, including household bills, change of address, access to a GP, welfare benefits etc
- Ensure exit strategies are integrated in support planning
- Liaising with MH Supported Employment Services to ensure that people have every possible opportunity to fulfil personal employment goals and be economically active
- Signpost and handover to floating support, community support services such as Futures in Mind, Adult Community Learning etc where appropriate in order that the person’s independence can be sustained
- Develop an understanding across partners and stakeholders of each other’s roles and responsibilities
*This role is not a clinical or social work role requiring clinical or social work input – this remains the responsibility of EPUT and ECC Social Care.
About You
- Educated to degree level/ NVQ 3 or have equivalent relevant experience in housing management.
- Experience of Providing advice and support on housing matters to individuals with mental health support needs.
- Experience of working with people with mental health needs and /or within the mental health sector would be an added advantage.
- Overview / understanding of housing law as applicable to this role
- Excellent communication skills with the ability to problem solve, negotiate and seek solutions to challenging and complex situations.
- A knowledge of person-centred planning to support the development of networks for people via the creation of support plans to maximise independence and wellbeing.
- Ability to work collaboratively, build positive relationships and networks across both internal and external teams and multiple stakeholders focusing on the best outcomes for the individual.

Please note that the job role requires you to be mobile throughout an operational area. Therefore, the post holder will need to have a driving licence and access to a vehicle, or the ability to meet the mobility requirements of the role through other means.
